Small critique
Shouldn't the categories be:
Forty-One to Forty-Five, Forty-Six to Fifty, Fifty-One to Fifty-Five, etc.?
The way it is now if you're on the fives or tens you have to choose between two categories.

I can't remember why I wrote "average".
Maybe I thought I would write "on average" because I never ask a preson their exact age (unless they know and trust me). I feel that people are more comfortable with the question: "What is your age range?"
For lots of reasons. Blurting out your exact personal ID info isn't always wise...Then there is Vanity.

I wasn't quite six when Kennedy (John) was killed. I have no idea where I was at the time. I do, however, remember that there was nothing good on TV for almost a week afterwards. I guess that's what you'll remember at six.
Hell, I barely remember where I was when Bobby Kennedy was killed, and I don't have a clue what I was doing when King was shot. And I was ten years old at the time.
